RULES & REGULATIONS FOR FELLOWSHIP

  You have to report to Hospital at 8.00am sharp till late evening daily.

Academic Head : Dr. S. Natarajan, Chairman & Medical Director

The Schedule is as follows:

Monday / Wednesday / Friday : OPD days

Tuesday / Thursday / Saturday: OT days

In Rotation

1st Fellow: Diagnostics

2nd Fellow: Diagnostics/ OPD            

3rd Fellow : OPD


I] General Rules:

v      Fellowship stipend will be Rs. 8000/- per month.

NO accommodation will be provided to you.

v      Have to submit the joining letter on first day

v      You will have two other accompanying fellow doctors.

v      Have to bring your White Coat and wear it during your duty hours in hospital premises.

v      Have to bring your 20D & 78D or 90D lens and also Scleral Depressor.

v      Available on telephone for emergency situa­tion.

v      Enter patients' medical records onto the computer and prepare thanks letter for doctors, case summary, etc.

v      The training being intensive, you are advised not to take leave unnecessarily for wedding, birthday, engagement parties etc. Total leave allowed: 15days

v      If you require leave for unforeseen circumstances, you will take prior sanction from Medical Director as well as from Managing Director.

v      To speak with all the patients politely, not to get involved in the arguments and unnecessary loose talk with the patients.

v      Doctors are requested to keep their belongings in the proper place provided to them.

v      Have to manage patients in his absence.

v      Clarifications if any, to be confirmed with Dr. S. Natarajan / other consultants.

II] OPD:-

v      You will be responsible for seeing all Dr. Natarajan’s / other consultant’s patients, before they consult with them on your name.

v      Whenever possible, advise the necessary investigations, so that when they reach Dr. Natarajan/other consultant, they will be fully worked up, so that they can give their opinion.

v      When in doubt, ask Dr. S. Natarajan / other consultants for advice.

v      Take care to see that handicapped patients, very old patients, SAPs, debilitated patients are seen quickly without much shifting & delay.

v      Prior to sending the patients to Dr. Natarajan / other consultant, the patients should be fully worked up for other cataract, glaucoma, opinions if neeeded.

v      Write your findings, diagnosis, & investigations, but do not communicate your opinion to the patient, Dr S. Natarajan / other consultant will be advising them on the course of Rx.

v      Utilizes the rest of the time for project work.

III] Diagnostics:-

v      Fellow in charge of diagnostics is fully responsible for the patient.

v      You are required to be standby for FFA. You should check the FFA files and accordingly supervise Medical Photographer.

v      You must see that the pre – angiography consent forms and that the evaluation forms are signed.

v      Oxygen cylinder must be checked once a week. The emergency tray must be checked once a month & must be kept within accessible reach.

v      In case of patients who are allergic to certain drugs & eyedrops. It is necessary to call the allergy specialist as a standby.

v      In an event of any type of reaction, the emergency management protocol is pinned up on the wall which is to be followed.

v      Diabetics (especially brittle diabetics, patients with nephropathy etc), heart patients, clearance from the physician is necessary and always remember to keep 25% dextrose as a standby in c/o hypoglycemic attacks.

v      In cardiac patients anesthetist will assist you, however the overall responsibility of the diagnostics room is yours.

v      In case any of the equipment require servicing or any problem arising in the management of patient inform Dr. S. Natarajan / other consultant & contact the concerned servicing agents and fill up the complaint register.

v      You will be required to fill up the investigation register each investigation ŕ complete the records. This is especially important to recover data, while making for scientific presentations and publication. This register will make your job easier.

v      When in doubt about the working of any instruments be sure to ask the consultants.

v      Reports must be sent to the respective consultants as soon as they are ready.

v      In case of any mishap in the diagnostics room, immediately inform Dr. S. Natarajan / other consultant.

v      Can utilize the rest of the time for project work.

IV] OT:-  

v      Fellow should be in OT 30 mins prior to the starting of the OT.

v      Work up patients who are getting admitted for surgery.

v      Check their physical fitness, investigations to be done before surgery as well as prepare retinal drawings. Patients may get admitted late in the evening but still pre-operative detailed retinal drawings should be done with international colour coding properly.

v      Pre-operative­ly you should explain to the patients the pros and cons about surgery and clear all their doubts.

v      You must inform Dr. S. Natarajan / other consultant in case of any delays morning investigations ordered for patients on the day of surgery.

v      You must be in OT, at the time of shifting of the patient, blocking (or GA) and must be washed up before Dr. S. Natarajan / any consultant is in the OT.

v      You must check that all instruments, autoclaving and equipment and sign in the corresponding register.

v      Responsible for assisting in surgery, as instructed by Dr. S. Natarajan / consultants.

v      After the surgery, you are responsible for completing post operative orders and registers.

v      You are responsible for seeing the post operative on the following day before Dr. S. Natarajan sees his patients. Post operative examination should include all details.

v      Post – operative dressing to be done and reported to the Surgeon.

v      Ensure that before discharge all formalities are completed. Make sure that Discharge Summary is made ready by the Secretary and initial it.

v      You should complete all medical records including admission records, operation notes, postoperative orders, etc.

v      You should also see all the set protocols are followed by nursing staff in Ward and O.T.

v      On OT days, it is the collective responsibility of all the fellows to see that preoperative diagrams and preoperative photos are taken, especially the fellow on duty in the OT the next day.

v      The files (Inv, consent, recent findings etc) should be checked 1 day prior.

v      The timings & order of the operations should be discussed with Dr. S. Natarajan one day prior.

  V] Diagnostics / OPD :-  

v      Fellow will be required to attend OPD & OT if there is any case in OT.

v      B- Scans will be done by this fellow.

v      Accommodates the duty of the absent fellow.  

VI] Academic:

A) Classes: -

v      Morning classes are conducted on Monday and Friday at 8. 30 am sharp for 1 hour.

v      To attend the classes and duty on time.

v      Fellows will be given their topics 1 month in advance. And in the case of journal clubs they will receive the topics 1 week in advance.  

B) Log Book:-

v      Log Book to be maintained and monitored weekly by the consultants.  

C) Library :-

v      All the Doctors are requested to maintain silence in the Library.

v      All are requested to sign the Library Register Book while taking and returning the book.

v      No books will be allowed to be taken out of hospital premises.

v      Books will be issued between 11.00am to 7.00pm.

v      Books should be returned before 7.00pm

v      If anyone requires books from the library for any preparation for Dr. S. Natarajan’s presentation or for any other purpose, please plan in advance and issue well in time.

v      All are requested to handle the books with care and not to leave them anywhere in the hospital premises.

v      All are requested to return the books within 15days from the date of issue.

v      Kindly see that the cupboards are locked properly after taking the book.

v      If “case file” for some reason are brought to the Library, they should be returned to their respective place, same time by the person concerned before you leave the library.

v      No meetings with the clients or medical representative in the library are allowed. Kindly take them to the waiting hall.

v      No personal discussions & chit chatting.

v      Only one book at a time can be taken.

v      No Lunch should be taken in library under any circumstances.

v      Also, the CDs from the CD Library are not allowed to take outside the hospital premises. It has to be handled carefully and returned back without any damage to the Librarian.

v      You have no permission to copy the data from the CD for your personal records.

D) Project work:-

v      All fellows are responsible for preparing Dr. S. Natarajan’s presentations both PowerPoint slides and text for various conferences. In addition to this, they are also responsible to help other Consultants with their Academic work.

v      Fellows should complete their project work, articles and send for publication.

v      Project work will be reviewed periodically.

v      To complete the allotted work like Research papers and presentations on time. No last minute hassle.  

E) Computer Facility:-

v      Consultant Computer is provided with the facility of 24hrs Internet, CD-Rom, Speakers, & Scanner.

v      All fellows have to strictly work on the Consultants Computer Only & not to use the Academic Computer.

v      In case they are using the Academic Computer they will be responsible for any loss of data being saved on Academic Computer.

F) On calls: -

v      One fellow will be required to stay on campus every Sunday from 9.30am to Monday Evening.

v      The duty roster will be prepared along with a consultant on call.

v      In the event of any emergency, the fellow on call must inform the concerned consultant / Dr. S. Natarajan.

VII) Charitable Camps :-

v      You will be required to go for camps organized by the Aditya Jyot charitable wings (Sion, Dadar & Mahalakshmi). This is done in rotation and will be informed accordingly.

v      Everyday you will man the free Vitreo Retinal Clinic.

VIII) Lunch Table Manners:-

v      NO lunch time as such and are expected to go only on rotations for a short break.

v      Lunch should be taken only in the place allotted (4th Floor - Terrace) and nowhere else.

v      NO permission will be given to have lunch after 4.00pm

v      Not more than 2 persons will have their lunch at a time.

v      Kindly maintain cleanliness and silence while eating.

v      Please ensure the newspapers are spread on the table before eating and thrown in the bin after eating.

GENERAL RULES AND REGULATIONS FOR THE EMPLOYEES

All staff should give utmost importance to the patient, their needs, guide them properly, talk to them politely and with courtesy. Those found with rude behavior or if any patient complaints about anyone, will be severely dealt with. 

>They are required to be punctual in their attendance. In case of emergency and if due to unforeseen circumstances they are unable to attend duty; the same should be communicated over telephone to Managing Director and not to anybody in Reception Counter. If Managing Director is not available, to the Medical Director or if both of them are not available, it is the duty of the employee to see that it is informed. Otherwise, it will be severely dealt with.

On resuming, leave application should be submitted for signature from Authority. More than three days sick leave should be accom­panied by Doctor's certificate. Before proceeding on long leave, prior sanction is to be obtained one month in advance. In case of emergency, already granted leave can be cancelled. Before going, alternate arrangements have to be made as to who will perform his/her duties during the absence. Charge is to be handed over to that concerned person. Half day and later comers should take signature on the application form.

Lunch time should not exceed 30 minutes.

Identity card should be displayed while on duty. Concerned staffs are required to wear proper uniform on duty. All are required to wear proper, neat and decent dresses. No shorts or Jeans allowed in Hospital premises.

Staffs are entitled to 21 days annual leave after the first working year. Prior sanction is to be obtained before taking leave, if leave is taken before and after Sundays or Public Holidays, the holiday will be considered as leave without pay.

It is the duty of all the staff to keep the Hospital premises clean.

All should get their Attendance Card signed by the Administrator or Accounts Department.

All hospital instruments/stationery are to be handled carefully. No documents, material or any other items belonging to the hospital should be taken out of the hospital premises without prior consent of the concerned authority. In case of damage/breakage, it should be entered in the book with signature. Failing to do so, strict action will be taken.

People in the concerned department, should take stock of the items kept there once in a month and should report immediately to the administrator. Any maintenance required for their department should be given in writing. Staff is at liberty to follow up the matter and see that the work is done.

No staff will be permitted to leave the hospital premises while on duty. In case they want to go out for official purpose, gate pass should be obtained. Also they are not supposed to send ward boys or others to bring food items or any other personal work.

Your appointment after confirmation is terminable at anytime by one month’s written notice on either side. The period of such notice count from the date it is given. If any employee leaves the job without one month's notice, he or she should forego the last month's pay and no certificate will be issued.

Strict disciplinary action will be taken against the staff devi­ating from the above rules and regulations.

When the employee leaves the job, dues will be settled only after handing over the work/duties to the person who takes over.

All  are  requested  in each department and  also  the concerned person  handling  the stock of that department to give  list  of stocks  on hand well in advance. Last minute orders will not be accepted from that department.

The reception and ward staff is responsible to co-ordinate with correspondence department for blocking of appointments

Schedule time to time during Sir's absence for attending future confer­ences, and also inform Sir accordingly.

No boy or anyone else should sit without work on counter or any department without any reason. If they are seen doing so, strict action will be taken.

Each department should have check-list of the work done in their department alongwith their representative and handover the same to the Administrator.

No boys should keep munching during consulting time or when tea is served. Also they should wash the tea cups and handover to the persons in charge.

Administration office will be open from 11 am to 1 pm. In case of any requirements or discussion for anything whatsoever, should be reported during these hours only. Nothing will be attended before and after these hours. In case of absence of Administrator, alternate arrangements will be made.

The stock room will be open only between 11 am to 1 pm.  No one should disturb or take anything out of time.

In case if your department is not clean, kindly feel free to report the same to the Administrator.

Every month staff meeting will be held, date and time will be decided as convenient to everyone. It is a must for all to attend without any excuse.

Salary will be paid on 7th of every month, in case a holiday is on 7th of the month, it will be paid on the 8th of that month.

Leave will be allowed only for a number of days sanctioned to them by the authority. If the leave exceeds the period specified, will not be allowed to join the duties.  Only in case of acute emergency one day grace will be permitted but only of a valid reason with proper supportive evidence is submitted to the authority. If not, strict action will be taken.

While Sir is away, the hospital will not remain closed.  There­fore, all staff members are requested to come and be on duty as routine hours. In case of failing to do so it will be calculated in a yearly leave.

All staff is requested to abide by the rules and regulations of the hospital which will be framed from time to time.  Rules are subject to changes.
